Practical support in the client’s own environment.
Logan includes established neighbourhoods, newer communities and homes with very different access and support arrangements. Seeing a client where everyday life happens helps the clinician understand the routines, barriers and strengths that may not be obvious in a clinic room.
A mobile assessment can support goals such as moving more safely at home, rebuilding confidence after illness, reviewing equipment, managing daily activities or following a clinician-directed therapy program. Recommendations are shaped around the person, their environment and the people involved in their care.
Our intake team coordinates Logan referrals by matching the requested discipline, the client’s location and the current clinical capacity. Referrers receive a clear response about suitability, information gaps and likely next steps.
